hm, hard do tell, maybe faulty card, maybe faulty card reader.
i would go for the faulty card.

If any of your friends have same card, try switching for few days(or whatever time is necessary for problem to occur) and see how it goes.

See i'm not sure if my post is relevant, but just advising you, Scan your Memory Card with a good Anti Virus via your PC or try Formatting it Via Your PC..... My Uncle had somewhat similar problem (he uses a Yari too) , all his Pics and videos would not open on his phone. so i copied the files onto my PC and formatted the mem card via my PC and copied the pics and videos back onto the mem card and everything was fine....
Sorry if my post is incorrect.... just trying to help

Try to format the memory card with fat 32.it works even if it is corrupted.

I'm confused this problem cause of bad microsd or card reader slot. the point is
1. this problem appear lately.
2. changing to new microsd, problem still happend, but not offen.
3. formating with fat32? still face same problem

I doubt it. As said before; it looks more likely to be the card reader on the phone thats the problem.
